Human Trafficking Victims
Instructions for submitting a request to block adverse information resulting from Human Trafficking
If you believe any information supplied by Clarifacts is included in your report due to Human Trafficking, you may submit a request to block this information by contacting us by telephone, fax, email or regular mail using the contact information below.

You will need to provide Proof of Identity, Proof of Victim Determination and Identification of Adverse Information, as follows, in order for us to process your request to block information:
Proof of Identity
- Full name (first, middle, last)
- Any other or previously used names
- Current address
- Date of birth
- Social security number
If needed, additional proof of identity may be required, including government-issued identification documents, utility bills, and/or other methods of authenticating personal identity.
Proof of Victim Determination
Documentation that shows you were forced to be a victim of Human Trafficking, which includes:
- A determination of Trafficking by a federal, state or Tribal government entity; or
- A decision of Court of competent jurisdiction establishing Trafficking, including papers filed in the case; or
- A finding of Trafficking by a non-governmental organization or human trafficking task force, including victim service providers affiliated with these entities authorized by a federal, state or Tribal government entity; or
- A self-attestation by you that identifies you as a victim of Trafficking that is signed or certified by a federal, state, or Tribal government entity, Court of competent jurisdiction or an authorized representative of these entities
Identify Adverse Information
You must identify the specific information you are requesting to be blocked. If needed, we can provide you with a copy of the last report we have on you. It may be that we have never provided a report on you, and if so, we will inform you that we have no information to block.
